New Sponsor Request Form

For existing FLP Distributor:

• A Novus Customer, who has been sponsored for six full months and has not achieved the level of Assistant Supervisor during this time, will be eligible to select a new Sponsor. The sponsor change will happen in the month, immediately after completion of these 6 months.
• The application for change of sponsor may take approximately 4 weeks to process.
• In the event that multiple IDs are found in the system, the multiple application case will first be resolved before processing the New Sponsor Request.
• It is mandatory to submit a self attested Identity & Age Proof alongwith this form.
• New Sponsor Request Forms accepted on or after 25th of the month will be processed in the following month.
• In case any of the above mentioned criteria is not fulfilled your New Sponsor Change form may get rejected.
